Principal Feedback - 2015

This year, Interim Superintendent LeBlanc has asked that all principals receive feedback on their job performance. As she works with each principal and provides supervision and evaluation, it is important for her to gather information from all stakeholders, so that she can help them to grow in their profession. Superintendent LeBlanc thanks you for your time and
participation in this survey.

Please use this form to evaluate the performance of the principal(s) in your building. Remember the purpose of this evaluation is to gather feedback. Feedback is anonymous and will be taken seriously. Please be honest and thoughtful.

Complete this form by checking the box that best fits the item. At the end of the form is a comment box. Note that you may fill out the form more than once (in case you are providing feedback on more than one person).

We understand that you may not be able to comment on all areas of this survey. Please feel free to mark "N/A" whenever you feel that you cannot make an informed judgment.

Please complete the survey no later than February 13, 2015. Paper copies of this survey are available in each school's main office. If not completing electronically, please drop off your survey to the Central Office at 157 Daniels Rd, Hardwick, VT 05843 or mail to Joanne LeBlanc, Interim Superintendent, P.O. Box 338, Hardwick, VT 05843.

When answering questions, use the following scale:
1 = Almost never
2 = Seldom
3 = Frequently
4 = Usually
